It's a fairly common one; I actually have a button on my mouse bound to flipping open my knife (whether it's selected or not) so I don't wear out my left hand.
I refuse to use
    Soda Popper
    Liberty Launcher
    Axtinguisher; Phlogistinator
    Stickybomb Launcher
    Tomislav
    Pomson 6000
    Quick Fix
    Bushwacka; Machina
    Enforcer; Spycicle

on CSn servers.
I don't expect you to, but I appreciate those of you that do.

I do it every time I'm about to get a kill or right after I change a disguise to make sure quickchange is always between revolver and knife, so I don't accidentally backstab some guy, and pull out a sapper and try throwing it at his medic's face.

Yeah, when you play spy, you kind of get into the habit of switching between them. I do the same after every kill just in case someone spotted me doing it. The sapper is always manually selected for me, unless I'm doing a stab 'n sab.

I have some binds that greatly streamline spying (and other classes as well). I have melee set to F for easy access, T set to friendly disguise, and scroll up set to primary, scroll down secondary. So I don't need to use the number keys at all to switch weapons (only to choose a class), and I don't have the possibility of overshooting with the scrollwheel. This way I also don't need to use Q which can be finicky especially when spying when you forget which weapon you last used. Just scrolldown to get the sapper, and I can overscroll without issue.

Yes it's a huge habit that a lot of spies have. I think it relates to doing so much weapon swapping with stabbing engineers and sapping real quick. Mine actually comes from CSS, I was an awper and if you quick switched to a different weapon and back it canceled the load animation and allowed you to snipe faster. People actually wrote scripts to do it for them, but I like having more control. I was also a sniper in tf2 when I first started and when you shoot you automatically scope back in, but if you weapon switch you don't scope in which allows you to gtfo.

and this. If you just changed disguises, your pda is your last weapon and it just won't switch to anything.
